چکیده

In 36 patients with the Wolff-Parkinson-White syndrome, ventriculo-atrial (VA) conduction was studied using the single test stimulus method. In 22 patients no significant change in VA conduction time occurred following test stimuli with increasing prematurity suggesting exclusive VA conduction by way of the accessory pathway, an accessory pathway with a shorter refractory period than the His-AV node pathway, or identical refractory periods in both pathways. In 14 patients showing this VA conduction pattern tachyeardias could be initiated by a single early ventricular premature beat. This finding lends credence to the hypothesis that the accessory pathway is an essential link in the tachycardia circuit. In two of the five patients showing exclusive VA conduction over the His-AV node pathway single re-entrant beats with antegrade conduction over the accessory pathway could be elicited by appropriately timed ventricular premature beats. In one patient showing this pattern a sustained tachycardia with AV conduction over the accessory pathway could be initiated. A total of eight patients did not show VA conduction over the

Ventriculo-atrial conduction time during reciprocating tachycardia with intermittent bundle-branch block in Wolff-Parkinson-White syndrome.

Records from patients with the Wolff-Parkinson-White syndrome were reviewed with particular emphasis on the occurrence of bundle-branch block aberration during reciprocating tachycardia and the significance of this observation with respect to accessory pathway location. Increase by greater than 25 ms in the ventriculoatrial interval during reciprocating tachycardia with bundle-branch block, whe...

Determinants of antegrade echo zone in the Wolff-Parkinson-White syndrome.

Forty-five patients with the Wolff-Parkinson-White syndrome and reciprocating tachycardia which utilized the atrioventricular (A-V) node for antegrade conduction and an accessory pathway for ventriculo-atrial (retrograde) conduction were studied. The extrastimulus technique in the right atrium and coronary sinus at multiple pacing cycle lengths was used. Three types of "echo zones" were defined...

Atrial reciprocal rhythm and reciprocating tachycardia in Wolff-Parkinson-White syndrome.

In an infant with type A Wolff-Parkinson-White syndrome, atrial reciprocal beats and attacks of reciprocating tachycardia were repeatedly recorded. Their dependence on a prolongation of the P-R interval could be well demonstrated during Wenckebach periods. Because of the normal aspect of the QRS complex during arrhythmia, the short ventriculo-atrial conduction time (0.08 sec), and the vectorial...

Mechanism of reciprocating tachycardia initiated during sinus rhythm in concealed Wolff-Parkinson-White syndrome: report of a case.

Reciprocating tachycardia in a patient with a leftsided atrioventricular accessory pathway (AP) (Kent bundle, type A) capable only of ventriculo-atrial (V-A) transmission is described. The V-A AP is established as an essential link of the tachycardia circuit, as evidenced by : 1) retrograde atrial activation of the left atrium (LA) 60 msec or more before the low and high right atrium during rec...
